(i) Sr. Program Officer (Procurement)-1, (ii) Sr. Program Officer (Reproductive Health)-1, (iii) Sr. Program Officer (Monitoring and Evaluation)-1, (iv) Program Officer (Clinical)-1, (v) Program Officer (Preventive Health)-1, (vi) Program Officer (Administration & Training)-1, (vii) Accounts Officer-1, (viii) Assistant Engineer-1 and (ix) Accountant-1 under Project Management Unit (PMU and (i) Monitoring and Quality Assurance Officer -14 , (ii) Accountant -3, (iii) Office Assistant cum-Computer Operator - 15, (iv) Driver-3 and (v) Office Attendant-10 under Project Implementation Unit (PIU) in different City Corporation/Pouroshava.. (1). The Firm shall be selected as per their experience, capacity (Physical and Financial) and evaluation of submitted CV for the positions. All information about the experience and other relevant information may be verified by the Project Authority if required, (2) The Firm must have experience of the 2 similar nature of service in the last five (5) years in the Government Sector and provide description of location, assignments, tasks, cost and duration of each assignment (3) Provide the description of general experience of other works in the past five (5) years including location, assignments, tasks, cost and duration of each assignment, (4) The Firm must have experience at least two (2) contracts of similar nature completed/ongoing over a period of five (5) years under Government Project or Sector where minimum one contract with a value of at least Tk 2,00,00,000.00 (Two Crore) shall be required, (5) The tenderer are given the preference who have own office set up in Dhaka, |

The firm must be submitted their relevant up-to-date registration certificate under the Joint Stock Company/FIRMBureau, Up dated Trade License (If any), VAT and Income Tax payment certificate, all experience Certificate, Bank Solvency Certificate, (7) The tenderer must provide the audit report for last three (3) years, (8) Manpower must be proposed for all position as per required qualification and experience of Appendix-C of the Tender Documents. If any tenderer shall not provide or propose for all position, that proposal will be rejected, (9) All interested firm shall be submit the Tk.5,00,000.00 (Five Lac) as Bid Security with the tender documents in the form of Bank Guarantee/Pay Order/Bank Draft from a Schedule Bank of Bangladesh in favor of “Project Director, Urban Primary Health Care Services Delivery Project-II”. Without Bid Security, the tender will be rejected, (10) No JV will be permitted. |
