Interested Consulting firms must provide the following information as a part of the EOI:
1) Relevant Trade license (Consulting firm) valid up to June 2024.
2) VAT registration certificate.
3) Income tax payment certificate for AY 2023-24.
4) Authority for signing the tender.
5) Joint Venture agreement (if applicable).
6) An Affidavit, issued not more than 30 days before the date of submission of EOI, with declaration of legal capacity, debarment history, litigation history & bankruptcy .
7) Organization Brochures describing its facilities and area of expertise
8) General experience of minimum 05 (Five) years (Years Counting backward from the date of publication of EOI)
9) Specific experience in government health education/promotion activities with successful completion of at least 01 (One) contract valued not less than -Tk. 1.75 Crore ( One crore Seventy Five lac) within last 03 (Three) years as a consulting firm. (Years Counting backward from the date of publication of EOI.)
10) Physical facilities (office, equipment, A.V. materials and transport).
11) Credit facility certificate from a schedule bank of Tk. 1.75 ( One crore Seventy Five lac) (Any credit facility certificate issued before 28 days prior to last date of submission of EOI will not be accepted).
12) List of available manpower, along with their Photograph & Phone number, describing their education qualification, designation in the organization as well as there area of expertise.

1) The Consulting firms should prepare and submit 03 (Three) copies (01 original & 02 Photocopy) of EOI in response to this advertisement. Each Copy of EOI shall be separately enveloped and all the Copies then be enveloped in one single envelop. Non Compliance of the above direction may lead to the rejection of the EOI.
2) EOI Must be Sequentially Numbered, Duly Signed/Initialed by the Competent authority. Non Compliance of the above direction will be considered as major Deviation hence will lead to the rejection of the EOI.
3) All the Legal documents/historical documents except Printed literature must be attested by any Gazetted First class officer.
4) For FY 2023-24 an applicant can’t submit EOI in more than 01 (One) package (Firms that have applied in any EOI launched by this PE during this FY, except for World Bank procurement, can not apply).
5) EOI can only be submitted at the office of the Line Director, Lifestyle and Health Education & Promotion, Bureau of Health Education, Directorate General of Health Services. Mohakhali, Dhaka.
