The applicant shall have to submit the EOI with the following documents: (1) Category of the firm: Consulting firm; (2) Brief description of the firm with proof of working in the relevant areas Health Guideline relevant Field; (3) Updated Valid Trade license/Registration; (4) Updated income Tax certificate; (5) Updated Certificate of VAT; (6) Minimum 05 (five) Years Experience in Health Guideline preparation related Firm; (7) List of major clients working in relevant area in last 5 (five) years; (8) Liquid money or credit facility certificate with of Tk. 50.00 (Fifty Lakh in case package AMC-2023-2024-SD-5 (Unani), Tk. 50.00 (Fifty Lakh in case package AMC-2023-2024-SD-6 (Ayurvedic) & Tk. 50.00 (Fifty Lakh in case package AMC-2023-2024-SD-7 (Homeopathic) (Any Liquid money or credit facility certificate issued before 28 days prior to last date of submission of EOI will not be accepted) (9) Updated Bank Solvency certificate with statement & Financial Audit Report (10) Name of the Central Key personnel with C.V. who will be involved with the assignment; (11) Relevant documents in favor of organizational strength of the farm. (12) Each Consulting farm can apply only in one Pacakge. |
