| Brief Description of Assignment |

Design awareness activities including development of messages on different health issues. Production of TV Spot, Drama, documentary, TV Scroll on different health related issues and telecast in different TV channels & social media. Arrange advocacy meeting ,workshop, seminar, training, community counseling, road show, folk song & any other enter educate activities, publication of newspaper advertisement, and design, development and distribution of SBCC material, research etc. & other SBCC activities. |

1.Organization Brochures describing its facilities and area of expertise.
2. General experience of minimum last 05 (five) years.
3. specific experience in government health education and promotion activities with successful completion of atleast 01 (One) single contract valued not less than tk. 1.4 crore incase of package LHEP-2021-2022-s-01;tk. 1.8 crore incase of package LHEP-2021-2022-s-02;tk. 1.8 crore incase of package LHEP-2021-2022-s-03;tk. 55 lac incase of package LHEP-2021-2022-s-04
4. availability of appropriate skills & qualification among the staffs, physical facilities (office, equipment, AV materials and transport)
5.Trade license valid upto 2021-22
6.Income tax payment certificate for FY 2021-22
7.VAT registration certificate
8.liquid money or credit facility certificate with of tk. 1.75 crore incase of package LHEP-2021-2022-s-01;tk. 2.5 crore incase of package LHEP-2021-2022-s-02;tk. 2.5 crore incase of package LHEP-2021-2022-s-03;tk. 65 lac incase of package LHEP-2021-2022-s-04 (any liquid money or credit facility certificate issued before 28 days prior to last date of submission of EOI will not be accepted.)
9. Name & CV of 5 professional staff & central key personnel describing their qualifications along with his/her last academic certificate, photo, contract number and email address who will be involved in the assignment. CV should be duly signed by the respective professional staff. |

1.Documents should duly sealed and initialed by the owner or his/her authorized representative.
2. EOI can only be submitted at the office of the Line Director, Life style and Health Education & Promotion, Bureau of Health Education, Directorate General of Health Services, Mohakhali, Dhaka.
3. The consulting firms should prepare and submit 03 (three) copies of EOI in response to this advertisement. any class one gazetted official must attest all the photocopy of the legal documents and certificates.
4. During the FY 2021-22 a firm can't submit EOI more than 01 (one) package. |
