INFORMATION FOR APPLICANT

| Brief Description of Assignment |
: |
- The consulting organization will conduct the national blindness and visual impairment survey in Bangladesh.
- This survey will provide data for planning and programme implementation towards elimination of avoidable blindness in Bangladesh.
- Finalized the survey proposal with a valid methodology. Proposal should have clear statement of research question and or hypothesis
- Recruit field surveyor personel with Ophthalmologist.
- Organize training of the staffs on the assignment and monitoring.
- Organize household survey and eye camp to determine prevalence and cause of blindness and impaired vision,
- Gender role an important concern and would be addressed at every step of research.
- Appropriate and standard statistical method would be used for data analysis and interpretation.
-Organize a dissemination workshop to disseminate the research findings.
- The consultant organization will design, implement, analyze and report the survey with internationally acceptable standard and quality.

The applicant shall have to submit the EOI with the following documents: (1) Valid Trade license /Registration; (2)Updated Income Tax certificate; (3) Certificate of VAT; (4)Category of the firm: Research organization/Survey organization/Consulting firm; (5) Brief description of the firm with proof of working in the relevant areas and of conducting survey; (6) Key expert of the team should provide: a) List of research conducted, b) List of publication;(7)10(ten) years experience in the relevant activities of survey/research; (8) List of major clients working with in last five years with contact reference; (9) Bank solvency certificate; (10) Name of the central key personnel with C.V. who will be involved with the assignment; (11) Relevant documents in favor of organizational strength of the firm 12) The organization should have experience in conducting nationwide survey.13) Organization should have experience in conducting blindness survey. 14) Organizations should have permanent public health expert (s) having doctoral degree and should have at least 20 publications in peer reviewed journal.
