| Brief Description of Assignment |

a) Design and finalize digital display board on GMP, IYCF, EBF, Adolescent and Maternal Nutrition, Elderly Nutrition, Nutrition Messages etc.
b) Build and install digital display board at district & upazila level.

| Experience, Resources and Delivery Capacity Required |

The applicant shall have to submit the EOI with the following documents:
1. Category of the firm: Consulting firm/Advertising Agency/Media development firm;
2. Brochure/ brief description of the firm;
3. Trade license /Registration valid up to 2016-2017;
4. Tax clearance certificate for the year of 2016-2017;
5. VAT registration certificate (last fiscal year);
6. At least 5 (Five) years similar experience in the relevant activities with completion certificate;
7. At least 3 (Three) evidence of specific experience in health and nutrition related activities with completion certificate;
8. List of major clients working with at present with their contact reference (mail, phone, mobile etc.);
9. Updated bank solvency certificate and credit facilities of Tk. 50.00 lac between July 2017 to February 2018.
10. Name of the central key personnel with C.V. who will be involved with the assignment duly signed by the respective personnel & authority;
11. Relevant documents in favor of logistics and organizational strength of the firm to prove your capacity to perform the assigned task;
12. Audited Financial Statements for last fiscal year by external Auditor(s).
13. One firm can apply for only one package mentioned above.
14. EOI must be submitted in sealed envelope separately for each assignment.
