(a) Bank draft/pay-order (original copy) for the prescribed tender in favor of Civil Surgeon cum Superintendent, 250 bed Modern District Hospital, Khagrachari. (b) Attested receipt of tender purchase. (c) Renewal of Trade License. (d) Current Income Tax Certificate paid by Mr. (e) VAT Registration Certificate paid by Mr. (f) 01 (one) color copy of passport size photograph of tenderer. (g) 03 (three) duplicate signatures on the contractor's own pad to be attested. (h) Experience certificate with memorial number on the concerned group in the Government Hospital in the institutional pad of the said hospital which should bear the signature and name of the Head of the institution and seal. (i) Financial solvency certificate and bank statement of the contractor for the current year in the own pad of any Scheduled Bank approved by the Government bearing the signature (with seal) of the Bank Manager. (j) A declaration/undertaking must be given in its own pad for not exerting any influence/lobby/recommendation by any prominent person or individual for obtaining the work order. (k) No one can take recourse of law against the Tender Committee which shall be deemed to be indmissible as per PPA-2006 and PPR-2008& Revised PPR 2009 & 2019. The tender should be submitted after duly filling in the attached tender form. (l) Membership certificate (updated) of Chamber of Commerce or Trade Association. (m) Birth Registration Certificate. (n) The declaration/undertaking to comply with all the terms and conditions mentioned in the tender and the schedule shall be given in its own pad.
(a) The authority reserves the right to accept or cancel any tender or all tenders without assigning any reason (b) Due to unavoidable reasons: If sale, acceptance and opening of tenders is not possible on the scheduled date, it shall apply on the next working day. (c) The schedule cannot be altered/extended by the tenderer. (d) The decision of the authority regarding the tender shall be final. (e) Work orders shall be issued subject to tender administrative approval from appropriate authority and bill shall be paid after receipt of expenditure sanction. (f) Emergency supply of Medicines and MSR equipments are essential items. Quotation of rates for the items mentioned in the Schedule is mandatory, no exceptions shall be accepted. |
