Must be a Bangladeshi Citizen and submit National Identity Card.
Electronic Tin (E-TIN) registration certificate.
Valid Trade License for the fiscal year 2018-19.
Income Tax Clearance Certificate for the Fiscal Year 2018-19.
Valid BIN Registration Certificate.
Bank Statement (Original copy) from 01-06-2018 to 31-05-2019. Bidders bidding for different groups should attach the original copy of the bank statement with one group and attested photocopy of the same with other groups.
Financial Solvency certificate stating amount from any scheduled Bank with the bidder holding an account (The statement should be issued after 31-05-2019). Solvency certificate should be based on Bank Statement attached. Certificate Stating “Sound & Solvent” is not acceptable and biddder submitting that type of certificate will be rejected.
Bidders of Group: 01 must be experienced in supplying at least 100 Cleaner to any at least 800 to 1000 bedded hospital and bidders of Group: 02 must be experienced in Washing dirty linen goods in any at least 800 to 1000 bedded bedded hospital within the last 03 (Three) fiscal years. Bidders of other groups must be experienced in supplying / performing similar goods / work to / at any Tertiary level Hospital / Specialized Govt. Hospital / Civil Surgeon Office / Any Government Hospital within the last 03 (Three) fiscal years. Experience certificate and copy of work order must be submitted. 02 (Two) Passport size photo of the bidder / representative.
